I made a CD cover for Mac OS X Leopard, including instructions in the case, it's very nice: download the cover here How to print: Use a label designer application (like Nero Cover Designer (Windows), DiscLabel or Toast Disc Cover RE (Mac)) to fit the image to real label size (12x12cm) and, print it.
